About Us

Vista Post Acute is a Medicare-certified skilled nursing home located in Hayward, CA. We are located on a hill with a view below our property of deer, turkeys, and horses roaming the fields and on a clear day, stunning views of the city of San Franciso and the bay. We have been serving our community since 2005 and are under new ownership with an entirely new management team.

We pride ourselves on the feedback we receive from our residents and they have told us that we cultivate a family atmosphere and a homelike environment with a dedicated staff that truly cares about the patients we serve. We provide great clinical outcomes with very low rehospitalization rates. Our Therapy teams strive to reach our patient’s therapy goals so they can discharge back to home or highest level of independence.
